Filing Guidelines
Please use these guidelines for efficient and secure claims filing.
NAME, EMAIL ADDRESS & CLAIM NUMBER
Provide your complete name, Email
address and any other
case-specific information that may be
requested. Be sure to include
any identifying number from the Notice
that is associated with your
case (if you received one), and if
requested during the submission
process. Once you receive a claim
number it should be used in any
correspondence submitted to Heffler
Claims Group.
SOCIAL SECURITY & TAX IDENTIFICATION NUMBERS
Many cases that include a
distribution require Heffler
Claims Group to verify claimant
information. Typically,
this is your Social Security (SSN) number or
Tax Identification
(TIN) number, which is also required for tax
reporting on federal
Form 1099. Your claim may be rejected if this
information is not
accurate or omitted. Please note that all such
confidential
information is maintained on a secure database for
internal use
only.
CLAIM FORM
Claim filing periods are typically
defined on the Claim
Form associated with your case. It is important
that you complete
and submit the claim form by the deadlines noted in
the form. Make
certain that the information on the claim form is
accurate and
legible or it could be rejected.
SUPPORTING DOCUMENTATION
Double-check that you have provided
comprehensive
documentation, if requested. Do not send originals
(unless
required), and maintain copies of each document you submit.

SIGNATURES
You must sign your own claim form. A
beneficial owner or
legal representative who is authorized to sign
for you must also
sign the claim form.
CLAIM SUBMISSION
Submit your claim by the deadline
indicated on the Claim
Form. If you would like a receipt, send via
Certified Mail/Return
Receipt Requested.
DISTRIBUTION TIMELINE
Please be patient. Cases have many
Claim Form
submissions that must be verified before distributions can
occur. It
is possible that additional data may be requested of you
after
receipt of the initial Claim Form, and before your claim can be
processed.
